So my state doesn't offer a scannable vaccination record with QR code (just a PDF that looks like it was homemade), but the supermarket where I got the vaccine does.

We're not even in the situation where we have 51 different standards for proof of immunization in the US, we've fallen back to letting the private providers maybe implement it for us (if they choose to).

At least many of the private pharmacies appear to be using a format that is compatible with the state of California and others. A good explainer on SMART health cards by @vishnuravi:

There's also an effort by Clear to become a digital passport provider, but there's some reason to be skeptical of giving that data to a company that has other interests.

Will any of these work when traveling outside the US? The EU and other countries have their own format, and of course it won't connect with the patchwork US system.

Of course, in the EU such things are protected by strong privacy laws including the GDPR. In the US, not so much.
